Missed Appointment Policy

  • Missed appointments will have same day follow up from your nutritionist.

  • If session needs to be rescheduled, it must be canceled within two hours of scheduled time without penalty.

  • If a RESCHEDULED appointment is missed again without atleast two hour advance notice, the client will lose the session.

  • After 2 no-show appointments (appointments missed without notice before the start time) will result in the forfeiture of all remaining sessions without refund. Your nutritionist reserves the right to refuse further service.

Subscription Policy

  • No refunds will be given after 48 hours of payment draft received. If a refund is granted within the 48-hour window, then no further sessions may be or remain scheduled beyond the cancellation date. Additionally, the price point that the client maintained will be subject to change to the most updated pricing.

  • All subscriptions will be drafted on the same date of the next calendar month. You may speak with your trainer about adjusting your draft day.

  • By signing this agreement, you understand all subscription packages will renew on the same date on the next calendar month unless canceled by the client or prior amendments have been made beforehand and trainer has agreed.

  • By signing this agreement, you understand refunds requested within the 48 time frame and granted, will take from 5-10 business days to process.

Tardiness Policy

  • Clients arriving late will receive the remaining scheduled session time unless other arrangements have been previously made with nutritionist. If you are more than 25 minutes late, your nutritionist may choose to cancel the session and you may lose that session, in which case no refund will be made.

  Expiration Policy

  • The expiration policy requires that the client books their sessions within 30 days from the date of purchase. Sessions are void after this time period. No refunds will be issued for unused sessions.

Testimonial and Photo Release Form

In good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I, the undersigned, hereby grant to BODY- Nutrition Services (“Nutritionist”) and his/her agents the right to use my photographs, images, story, and/or testimonial, in whole or in part, and without restriction as to changes or alterations. The rights granted herein shall extend in perpetuity, unless revoked in writing to Nutritionist by me, throughout the world, and for any purpose whatsoever, including without limitation for marketing and advertising purposes of Nutritionist, and in any and all media, including without limitation Trainer’s website. I acknowledge that Nutritionist has no obligation to return any photographs or images to me. I hereby RELEASE, WAIVE, and FOREVER DISCHARGE any and all claims arising out of, or in connection with, such use by Trainer, including without limitation any and all claims for libel or invasion or privacy. I hereby warrant and represent that I am at least 18 years of age and have the right to contract in my own name. I have read the above Release and am fully familiar with the contents thereof. This Release contains the entire agreement between the parties hereto as to the subject matter contained herein.
